Conference and Travel Expenses Form
The Expenses Claim Form may be downloaded and returned to the WLUFA Office. Please use this form for reimbursement of expenses for pre-approved conference fees (travel, per diem, etc.).
The APPLICATION FOR MEMBERSHIP form may be downloaded and returned to the WLUFA Office. All faculty are members of the bargaining unit, you must complete this form to become a member of WLUFA.
